Young Adult Eligibility Documents

You must provide documentation for one of these categories at the intake session,

Participant

  • Current Photo ID- State ID or Driver’s License or Passport (Please provide one of these three options.)

  • Birth Certificate

  • Signed Social Security Card

  • Proof of Income IF EMPLOYED

    • 2 most recent paystubs, or 2021 W-2 or 2021 1099 (Please provide one of these three options.)

 

If no Income (Parent provides proof of Income)

  • Parent Name and Date of Birth

  • Parent Social Security Number

  • Proof of Income

    • 2 most recent paystubs or 2021 W-2 or 2021 1099 (The parent only needs to provide one of these three options.)

 

Other members of your household under 18

  • Birth Certificate 

  • Social Security Number
