Intake Appointment: Documents Checklist

DOCUMENTS MUST BE DATED WITHIN 60 DAYS OF YOUR SCHEDULED APPOINTMENT.

  1. Proof of Your income –- See chart below for income guidelines.

    • If you work: 2 most recent pay stubs, W2 from 2022, or IRS Tax Transcript from 2022

      • I-9 is not accepted

    • If you receive Worker’s Compensation or Unemployment Benefits, please bring award letter

    • If you do not have any income: You must show proof of how you support yourself and your family (cash assistance, Social Security benefits, etc.)

    • A letter that states your family receives SNAP (food assistance)

  2. Proof of Identity and Residence (parent / guardian) – ALL following documents are required.

    • Your current, unexpired State Issued photo ID

    • Proof of current residency in Cuyahoga County and/or the City of Cleveland

      • If the address on your ID is where you currently live, we are good to go.

      • If not, you will need one of the following: utility bill, government agency letter, or lease agreement that is less than 60 days old at time of your appointment.

    • Your Social Security Card (letters WILL NOT BE ACCEPTED)

  3. Documents required for the teen (who will be working in the program)ALL following documents are required:

    • Your child’s current, unexpired photo ID

      • 14 – 17 years old: School ID (from 2021-2022), School Records with Photo, State ID, Driver’s License, Passport, or City Rec ID

      • 18+ years old: Driver’s License, State ID, or Passport

      • If the expiration date on a Driver’s License or State ID is March 9, 2020 to April 1, 2021, the expiration date has been automatically extended and will remain valid until July 1, 2021.

    • Your child’s Social Security CARD (letters WILL NOT BE ACCEPTED)

    • Proof of Relationship to the child wishing to participate.

      • A Birth Certificate often satisfies this requirement if you are listed as the parent.

      • If the Birth Certificate does not show proof of relationship between you and your child, you can provide school records, custody papers/court documentation, or medical records.

    • If your child is 18 years old and currently in High School: Copy of their current report card or proof of current year registration

  4. Required documents for other family members

    • You may be requested to provide documentation for others in your household if you are only income eligible with more family members.

      • For these family members you need to bring ALL the following information:

        • If another minor:

          • Proof of relationship between you and the child (example: birth certificate or school records)

          • The child’s Social Security Number

          • The child’s Date of Birth

        • If another adult:

          • 2 most recent pay stubs, W2 or IRS Transcript from 2022, or attestation to no income

          • Social Security number

          • Date of birth

Income Guidelines for Families Qualifying based on employment income, worker’s compensation, or unemployment benefits – Income must be no more than 200% of Federal Poverty Level.
